Young’s Lodge on the Chariton COVID-19 Liability Waiver
I acknowledge the contagious nature of COVID-19 and that the CDC recommends getting vaccinated, wearing a mask, staying at least 6 feet away from others, and washing hands often.
I voluntarily choose to lodge at and/or participate in any events held at Young’s Lodge on the Chariton and acknowledge I’m increasing my risk of exposure to COVID-19.
I further acknowledge that Young’s Lodge on the Chariton has put in place preventative measures to reduce the spread of COVID-19 but cannot guarantee that I will not become infected.
I understand the risk of becoming exposed to COVID-19 may result from the actions or negligence of myself and others, including, but not limited to Young’s Lodge on the Chariton staff, volunteers, vendors, musicians, and other guests.
I attest that:
- I’m not experiencing any symptoms such as cough, shortness of breath, fever, chills, shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- I haven’t traveled internationally within the last 14 days.
- I haven’t traveled to a highly impacted area in the United States within the last 14 days.
- I don’t believe I’ve been exposed to someone with a suspected and/or confirmed case of COVID-19.
- I haven’t been diagnosed with COVID-19 within the last 14 days.
- I’m following all CDC guidelines as much as possible and limiting my exposure to COVID-19.
Young’s Lodge on the Chariton Photo Release
I hereby grant Young’s Lodge on the Chariton Fest permission to use my likeness in a photograph, video, or other digital media (“photo”) in any of its publications without payment or other consideration.
I understand and agree that all photos will become the property of Young’s Lodge on the Chariton.
I irrevocably authorize Young’s Lodge on the Chariton to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to approve the finished product wherein my likeness appears. I waive any right to compensation related to the use of any photos.
I release and agree to hold Young’s Lodge on the Chariton harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the festival, or that may otherwise arise from my lodging at and/or participating in events at Young’s Lodge on the Chariton. I understand this release discharges Young’s Lodge on the Chariton from any liability or claim that I, my heirs, or any personal representatives may have against the establishment, and its staff with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to lodging at and/or participating in events at Young’s Lodge on the Chariton.
